Overview
The Mental Health Nurse Program is a free, Federally funded program run through Richmond Futures (RFT), providing evidence-based mental health nurse support in the community. The program is available to Huon Valley residents and beyond, for adults with a chronic or complex mental health presentation.
The nurse provides assessments, therapeutic interventions, medication monitoring, mental state assessment, family support, care coordination, and referral to appropriate services. The program is targeted to patients with a mental illness who are currently being managed by their GP or private psychiatrist.
Access to the program is by referral — please speak to your GP about a referral, with further information and referral forms available on the website, or call the office to discuss.
